Perms and smoothing treatments are chemical services, so I need to know your hair's history before we meet. Please fill this out on your own time before booking. It takes about five minutes.

Be as honest and specific as you can. Past color, bleach, box dye, or a previous perm or keratin treatment can change what's safe and what's realistic for your hair.

I understand perms and smoothing treatments are chemical services that change the structure or behavior of my hair.

I understand possible risks include irritation, allergic reaction, dryness, breakage, changes in curl pattern, or an uneven or unexpected result.

I understand results depend on my hair's history, condition, and home care, and no particular result or duration is guaranteed.

I understand a strand or patch test may be required, and Lulu may postpone, modify, or decline a service if my hair or scalp isn't suitable.

The information I've given is accurate, and I'll tell Lulu about any changes before my appointment.

I have read the reservation policy. Cancellations or reschedules must be made at least 24 hours before my appointment, or I'll be charged 100% of the service price. Arriving 10 or more minutes late, or rescheduling the day of, is treated as a no-show and charged at 100%. The $25 consultation fee is nonrefundable and nontransferable.

We'll go over the plan, risks, cost, and aftercare together at your consultation, and you'll sign final consent in person before any chemical service.
